- 80% Co-Insurance
- $35 CoPay for Physician Office Visits including Preventive
visits (unlimited # of visits)
- Preventive Services (Lab testing, Immunizations, Mammograms,
etc) covered at 80% Co-Insurance, not subject to deductible
- $35 CoPay for Non-Specialist Office Visit (unlimited
# of visits)
- $35 CoPay for Specialist
Office Visit (unlimited # of visits)
- 80% Co-Insurance for
ER
- $10 CoPay for Eye Exam (one exam per person every 12
months)
- $7 million lifetime maximum benefit
- Maternity Coverage Optional (80% Co-Insurance for physician
care, $3000 CoPay for Hospital facility)

Prescription Drug Card featuring:
- $15 CoPay (or 40%, whichever is
greater) for
generic drugs with NO drug deductible
- $15 CoPay (or 40%, whichever is greater)
for brand name drugs after you meet a separate $250 drug deductible. Plus difference in allowable charge
if Brand is chosen over an available generic. Out of pocket maximum $300 per prescription
and $4,000 per person per calendar year
